La maggioranza dei problemi etici implicati nella terapia di sostegno delle funzioni vitali possono derivare da una diversa concezione delle competenze in materia di salute e da una non univoca interpretazione del principio di proporzionalità. Soprattutto negli ambienti anglosassoni si afferma la teoria che l'autorità nella cura medica competa esclusivamente ai pazienti o ai loro rappresentanti legali.
